Home and Community-Based Services (HCBS) are designed to reduce the risk of out-of-home placements for children and youth through the addition of home / community supports that increase the functional levels of the youth and family. These intensive services require participation in Children’s Health Home Care Management Services and Level of Care determination prior to enrollment.
About this Program
Hillside’s Home and Community-Based Services make a number of supports available to eligible children, youth and families based on family preference and Level of Care determinations. These include:
- Caregiver / Family Advocacy and Support services enhance the youth’s ability to function as part of a caregiver/family unit and enhance the family’s ability to care for the youth in their home and community. These services assist all involved in understanding and addressing the needs of the participating youth relative to any disabilities they may have.
- Planned Respite and Crisis Respite services provide structured short-term relief for families / caregivers that better enables them to support the child’s functional, mental health/substance-use disorder, developmental, and/or health care issues. Respite supports can be provided based on anticipated or planned needs, or in consideration of an unexpected need during a crisis situation.
Eligibility
Open to youth ages 0-21 years. Must be a Medicaid, Medicaid Managed Care or Medicaid Family of One recipient. Must meet Level of Care requirements. To make a referral, contact info@hillside.com.
These services are open to eligible youth in the following counties*: Allegany, Cattaraugus, Chautauqua, Cayuga, Chemung, Cortland, Erie, Genesee, Livingston, Madison, Monroe, Niagara, Onondaga, Orleans, Oswego, Schuyler, Steuben, Wayne and Wyoming.
